It depends on the modifications:
1. If, in the settings of the Opt-In Form on the Coaches Console end, you change the fields for the subscribers to complete (first and last name, email address, phone number) and the consent check-boxes, then you do have to update the embed code of the Opt-In Form on your external website.
2. The modifications that you make in the Console Opt-In forms in terms of Thank You page or autoresponder messages (changing, adding, deleting) after you place the form code on your external website WILL BE UPDATED AUTOMATICALLY. You don't have to replace the Opt-In Form embed code with a fresh one on your external website.
