You don't need to change the Call Strategy type of the form, especially if this is the form you want your clients to fill out before an appointment (the form associated with the appointment notification/confirmation and reminder).
It is enough that you change the form's name in WEBSITE > FORMS. Click on edit from the drop down Options list to the right of the form and change the title with the one you want. Save the modification.
Then, to change the Call Strategy link message in the appointment confirmations (notifications) and reminders, you can go to My Account (the link in the upper right corner of your coach screen) > "Notification Templates" > "Appointment Confirmation", respectively "Appointment Reminder", delete the replacement tag $CALL_STRATEGY_LINK$, that automatically displays the message with the link to the Call Strategy link, and replace it with a link of your own.
You can write something like "Click here to fill out the Session Preparation form", select with the cursor the linkable text, click on the "Insert/Edit Link" and choose the internal link to the form from the Link List.
Click Save.
